진료과소개

종양혈액내과는 각종 악성 종양(암)을 진단하고 치료하는 내과로 크게 종양내과와 혈액내과로 나눌 수 있습니다.
고형암의 경우 수술이나 방사선, 항암치료를 할 수 있는데 혈액종양내과는 이중 항암치료를 전문적으로 다루는 과입니다.

주요질환

◎ 혈액내과: 급성백혈병, 만성백혈병, 다발성 골수종, 림프종, 진성적혈구증가증, 본태성 혈소판증가증, 골수이형성증후군, 혈소판감소증, 빈혈, 혈우병 등의 출혈장애
◎ 종양내과: 식도암, 위암, 대장암, 폐암, 유방암, 두경부암, 간담도암, 췌장암, 육종, 자궁경부암, 난소암, 신장암, 방광암, 전립샘암, 갑상샘암 등.

주요시술

◎ 항암제 치료
항암제는 투여방법에 따라 크게 먹는 약과 혈관 주사로 주입하는 주사제제로 나눌 수 있고, 항암제의 종류에 따라 투여방법이나 투여에 소요되는 시간, 그리고 부작용에 차이가 있습니다. 뇌나 척수 등의 중추신경계에 암이 전이된 경우에는 뇌척수액이 있는 공간에 항암제를 투여하는 경막내 화학요법 (Intrathecal chemotherapy)을 시행하기도 합니다.
◎ 조혈모세포 이식
이전에 흔히 골수이식이라고 불리던 치료로 고용량의 항암제를 투여한 후 골수나 말초혈액에서 얻은 조혈모세포(피를 만드는 세포)를 주입하는 시술입니다. 주로 급성백혈병이나 골수이형성증후군 같은 혈액 암의 치료에 사용되고 있으며, 조혈모세포를 얻는 대상에 따라 자가조혈모세포 이식 (환자 자신에게서 얻는 경우), 타인조혈모세포 이식 또는 동종조혈모세포 (면역학적으로 비슷한 타인에게서 얻는 경우), 제대혈조혈모세포 이식 (신생아 출산 후 태반조직에서 얻는 경우)으로 나눌 수 있습니다. 조혈모세포를 이식하는 과정은 일반적으로 수혈을 받는 과정과 크게 다르지 않으며 정맥주사를 통해 조혈모세포를 환자의 몸 안으로 주입하게 됩니다.
◎ 대면 다학제 진료
각종 고형암 뿐아니라 혈액암 분야에서도 병리과, 영상의학과, 외과 및 방사선종양학과의 전문의가 함께 모여 최적의 치료방침을 논의하는 진료 시스템입니다. 한 번의 방문진료로 여러 전문의의 의견을 듣고 통합적인 치료계획을 수립하여 환자 개인 맞춤 치료를 제공하여 암의 완치율 및 치료성적을 높이고 있습니다. 환자 본인 및 가족이 함께 진료에 참여하여 치료방법 결정에 동참함은 물론, 각 전문의와 상담이 가능합니다.
◎ 조혈모세포 이식

주요검사

골수검사: 백혈병, 다발성골수종, 재생불량성빈혈 등 골수에 일차적으로 생기는 질환을 진단하고, 치료를 진행하면서 치료에 얼마나 반응하는지 평가하는 것뿐만 아니라, 고형종양이 골수를 침범했는지 여부를 확인할 때 시행하는 검사 방법입니다.
