Cautions in issuance of copies
- The copy of patient medical record should be issued the patient oneself and a person with consent or power of attorney as it is personal data containing contents related to treatment of the patient and the consent and power of attorney should be completed on the contents (access and copy issuance) and autographed.
- When the above documents are prepared and submitted to the Health Medical Information Team, issuance of the copy is allowed.
